The number of chart-abstracted measures you are required to abstract and submit varies by which programs you qualify for and which certifications your organization wants.
Keeping all of the measures straight can be a nightmare. Today, we provide you two resources. One visual to help you remember the programs/certifications and measures associated with each. The other is an excel sheet with the full list of required measures by program/certification. Feel free to customize that excel sheet of measures according to the programs/certifications relevant to your organization.
