What does an ACO send to CMS?

An Accountable Care Organization (ACO) submits a defined set of data and documentation to CMS so it can verify quality, cost efficiency, and program compliance. In practice, this includes quality/performance reports, beneficiary notifications, enrollment or renewal materials, signed participant agreements, and updates that reflect organizational changes (e.g., mergers or new participants).

Data and Performance Reporting

ACOs provide standardized measures that show how they manage care and cost trends, such as:

  • Electronic Clinical Quality Measures (eCQMs)

  • Utilization and spending patterns

  • Readmission and complication rates

  • Outcomes and patient experience indicators

CMS uses these data to benchmark performance, determine shared savings/losses, and monitor continuous improvement.

Beneficiary Information

Assigned Medicare beneficiaries must receive notices that:

  • Explain the ACO’s role in coordinating care

  • Describe how information is used to improve quality

  • Clarify network and participation details and patient options

Application, Renewal, and Participant Agreements

When an ACO applies or renews (e.g., under MSSP), it submits:

  • Required program forms and operational details

  • Repayment mechanism documentation (where applicable)

  • Signed participant agreements for all ACO participants/suppliers

  • Change updates (e.g., participant adds/removals, corporate actions)

These materials confirm accountability to CMS rules and keep the ACO’s file current.
