Enrollment Overview

For Providers:

To participate in a CMS Accountable Care Organization (ACO), your organization must apply through CMS’s established application cycle. This process is managed through the ACO Management System (ACO-MS) and guided by the Application Guidance & Toolkit, which outlines eligibility criteria, submission requirements, and key deadlines.

Steps include:

  1. Reviewing the CMS Shared Savings Program Application Guidance & Toolkit.

  2. Submitting required documents and organizational details through the ACO Management System.

  3. Following CMS’s timeline for application review, approval, and participation agreement execution.

If questions arise during the process, visit CMS’s Contact Information page or the Shared Savings Program Application webpage for support.

For Beneficiaries:

You don’t need to sign up for an ACO plan. CMS automatically assigns beneficiaries based on their primary care provider’s participation in an ACO. This assignment may appear in your Medicare communications or on Medicare.gov under your account details.
