Who Can Report ACO Data to CMS?

Accountable Care Organizations (ACOs) are responsible for submitting aggregated performance and quality data to the Centers for Medicare & Medicaid Services (CMS). Rather than requiring each provider to report individually, the ACO, through its designated reporting entity, handles submissions for all participating clinicians.

Data Reporting Responsibilities

Authorized representatives

An ACO may use:

  • An internal quality reporting/analytics team, or

  • An authorized third-party vendor to manage data collection, validation, and submission.

Whoever submits must follow CMS-approved methods and ensure the data are complete and accurate.

  • CMS Web Interface – A secure CMS portal that many ACOs use to report population-level measures.

  • APM Performance Pathway (APP) – A standardized path aligned with MIPS for ACOs participating in the Shared Savings Program.
