What is a Good MIPS Score? | Medisolv

MIPS scores can range from 0–100 points. A good MIPS score is one that meets or exceeds the performance threshold that is set by CMS each year. In 2024, CMS set the MIPS performance threshold at 75 points. Clinicians who scored 75 points or higher qualified for a positive payment adjustment on their Medicare fee schedule.

How your MIPS score is calculated

MIPS program requirements are split into four categories: quality, promoting interoperability, improvement activities, and cost. Within each category is a set of measures. Your performance in each of these measures factors into your total score for that category. See what measures are in each MIPS category now.

Once your category scores are finalized, they are weighted and then added together to arrive at your final MIPS Composite Performance Score (CPS). CMS often modifies the category weights from year to year. In 2024, the category weights were as follows:

• Quality – 30%
• Cost – 30%
• Promoting Interoperability – 25%
• Improvement Activities – 15%

Qualifying for MIPS penalties & rewards

Your final MIPS CPS will determine the payment adjustment you receive on your Medicare fee schedule. Penalties and rewards are distributed on a sliding scale that’s based on how far below or how far above you score against the MIPS performance threshold that CMS has set for that performance year.

In 2024, the MIPS performance threshold was 75 points, so penalties and rewards were distributed as follows:  

• 0-18.75 Points: -9% penalty

• 76-74.99 Points: -8.99% to 0% penalty (depending on your score)

• 75 -100 Points: up to a 9% increase (depending on your score). To fund these rewards, CMS takes the funds set aside for those who did not meet the threshold and distributes them across those who did.

Understanding your scoring options

If you struggle to maintain a good MIPS score, keep in mind that MIPS is not the only reporting and scoring framework available to you under the Quality Payment Program (QPP). CMS has two additional frameworks — the MIPS Value Pathway (MVP) framework designed to serve specialists better and the APM Performance Pathway (APP) framework for advanced payment models like ACOs.
